The FDA wants feedback about your LASIK experience! Since this is the FDA, the body responsible for approving LASIK in the first place... your letter really WILL be 'feedback'. And the FDA link is a closed communication, so you can NAME YOUR DOCTOR AND CLINIC.

The site couldn't be simpler. They want your first name, last name, e-mail address and there's a box for you to type your story. Hit the 'submit information' bar, and you're done!
